Providing Facts

  • You need to provide the basic facts: time, date and location (be specific; write the exact location if possible) your number and security license number and names of any other staff present and names of any emergency service personnel who attend to the incident.
  • Describe how you where brought to the attention of the scene? Where called over the radio by management or another security officer? Did a patron alert you to the incident?
  • If you received a call over the radio you need to note what time you received that call.
  • You need to write a factual description of the incident describing what occurred. Stick to the facts do not write what you think?? Occurred.
  • Incident checklist:
  •  record the time day, date of incident
  • What type of Incident
  • Summary (description of incident facts, witnesses names, if persons injured, if any damage was done, names of victims and offenders, Offenders or suspects descriptions, names & License numbers of security attending and managers full name who was in attendance or reported to.
  • Action taken: YOU MUST LOG: Was person treated with first aid and who by?  Was the person offered medical assistance and declined? Was an ambulance called? Were police called and record their names and if given an event number record?  Names of supervisors and managers informed.  Any police action infringement notice issued or arrested. If police enquiries being carried out as no decision, document exactly that.
